What is Dental Prescription Maker?

Dental Prescription Maker prints a prescription that looks like the pad on your chamber desk. Enter your clinic once — clinic name and logo, the dentist's name, qualifications and registration exactly as you are entitled to print them, the chamber address, visiting hours and the standing advice you give after an extraction or a filling — and let this device remember it. For each patient, type the name, age, sex and date, tick the chief complaints, diagnosis, X-ray, treatment and systemic conditions in the left column (or write your own), and write the medicines you prescribe in the large Rx area: name and strength, dose, route, how often, how long and any instruction. Add advice, the next visit and an optional tooth chart in FDI, Universal or Palmer numbering, then download an A4, A5, Letter or custom-size PDF and PNG. The tool never suggests a diagnosis, medicine or dose, and the patient's details never leave the browser.

Key features

  • Two-column prescription pad: clinical checklist on the left, a large Rx area on the right
  • Your branding: clinic name, tagline, logo, dentist, qualifications, registration, chamber, hours, colour
  • Patient line with name, age, sex, date, reference number and next visit
  • Chief complaints, diagnosis, X-ray (OPG, IOPA, bitewing, CBCT), treatment and systemic conditions with tick boxes
  • Medicine lines with strength, dose, route, frequency, duration and instruction — written by the prescriber
  • Optional tooth chart: FDI, Universal or Palmer; permanent or primary teeth; condition, planned and done
  • A4, A5, Letter or custom pad size; single-column layout; or content only for a pre-printed pad
  • Faint watermark, footer advice on every page and page numbers when the prescription runs long
  • Bangla or English labels with Bangla digits and an embedded Bangla font
  • Several chamber profiles and your own saved medicine lines and advice, kept only on this device

Popular uses

  • Write daily prescriptions in a dental chamber or clinic
  • Print on A5 prescription paper or onto your existing pre-printed pads
  • Record the teeth treated and the treatment plan with a printed tooth chart
  • Give patients a clean, readable prescription in Bangla or English

Supported formats

PDF (A4, A5, Letter or custom size) and PNG of the first page

Troubleshooting

  • The printout is smaller or shifted Print the PDF at 100 % or “actual size”, not “fit to page”. For a pre-printed pad, adjust the top and left offsets.
  • Bangla letters look wrong The PDF carries its own Bangla font. If a printer driver substitutes fonts, print the PNG instead.

Also known as: Dental Prescription Pad, Dentist Prescription Format, Dental Rx Maker, Dental Chart Maker, Odontogram Maker.

How it works

  1. Enter the clinic and dentist once — name, qualifications, chamber, visiting hours, logo and footer advice — and let this device remember them if you like.
  2. Type the patient's name, age, sex and date; tick or type the complaints, diagnosis, X-ray, treatment and systemic conditions.
  3. Write the medicines you decide on — name and strength, dose, route, how often and how long — and the advice and next visit; mark teeth on the chart if you want it printed.
  4. Check the preview, download the PDF or PNG and print at 100 %.

FAQ

Is patient information uploaded or saved?

No. The prescription is made in this browser tab; the patient's details are not sent to Fah Swe servers or to any third party and are not saved. “New patient” clears them. Only the clinic details are remembered, and only if you tick “Remember”.

Does the tool suggest medicines or doses?

No. Diagnosis, medicines, doses and treatment are written by the prescriber. The pick lists are only the headings of a printed pad; nothing is suggested or checked.

Can I print on my pre-printed prescription pad?

Yes. The “Print on my pre-printed pad” layout prints only the patient and the prescription; move them with the top and left offsets to fit your header.

Which tooth numbering does the chart use?

FDI (ISO 3950), Universal (ADA) or Palmer, for permanent and primary teeth.

Is patient information uploaded or stored?

No. The prescription is made in this browser tab and the patient's details are not sent to Fah Swe servers or to anyone else. They are not saved either: press “New patient” or close the tab and they are gone. Keep the PDF as your record.

Does the tool choose medicines, doses or a diagnosis?

No. Everything clinical is decided and typed by the dentist or another authorised prescriber. The tick boxes are the headings of a printed dental pad; the tool does not suggest, complete or check medicines or doses.

Can I put my own clinic name, logo and registration on it?

Yes — and nothing is filled in for you. Enter only the qualifications and registration you are entitled to print. You can keep several chamber profiles on one device.

Can I print onto my existing pre-printed pad?

Yes. Choose “Print on my pre-printed pad”: only the patient line and the prescription are printed. Set the top and left offsets in millimetres until it lines up with your printed header, then print at 100 %.

Which tooth numbering systems are supported?

FDI two-digit numbering (ISO 3950), the Universal system (1–32 and A–T) and Palmer quadrant notation, for permanent and primary teeth.

What if the prescription is long?

It continues on another page with the clinic header and patient line repeated, and every page is numbered, so no medicine line is cut off.
